Good question, and I’m afraid a don’t know the answer. From memory, when I did mine, they didn’t ask for any witness details or proof of ID etc as I recall – but I could be mistaken, as it was 5 years ago. I did have my witness with me as it happens, but I’m not sure that was strictly necessary.
